What a near miss is, and what it is not

In each case the outcome was harmless, but the difference between the near miss and a serious incident was luck, not control.

The practical distinction is this: a hazard is a condition, a near miss is an event, and an incident is an event with a consequence. The same underlying weakness can present as all three over time. The value of near-miss reporting is that it surfaces that weakness while the consequence is still zero.

Why near misses matter

The single most effective way to increase near-miss reporting is to reduce the effort it takes to the absolute minimum. A worker who has just seen something unsafe should be able to report it in under a minute, from where they are standing, without leaving the operation.

Low friction by design

  • Mobile capture. Reporting should be possible from a phone or tablet at the point of work. A worker on the quay, in a yard or on a vessel should not have to return to an office.
  • Short forms. Ask only for what is needed to act: what happened, where, when, and how serious it could have been. Detailed analysis is the job of the reviewer, not the reporter.
  • Photographs. A single photograph often communicates a hazard faster and more accurately than a paragraph of description. Make image capture a core part of the report.
  • Plain language and local languages. Port workforces are multilingual. Forms and categories should be available in the languages actually spoken on site.

A just culture means that honest reports of errors, near misses and unsafe conditions are treated as opportunities to learn, not as occasions to assign personal fault.
